To answer your first question: Runner's knee and osteoarthritis are distinctly different types of problems. Runner's knee is a softening and inflammation of the cartilage underneath the knee cap, but at it's heart, runner's knee is a problem with the articulation between the patella and the end of the femur. This is more appropriately called a patellofemoral pain syndrome. It usually comes from bio mechanical imbalances between the hip and the knee. Osteoarthritis, on the other hand, is a degenerative process that is characterized by a thinning of the cartilage, sometimes to the point that the bone is fully exposed in the joint (ie "bone on bone"), osteophyte (bone spurs) and cysts that form within the joint. We don't fully understand the causes of OA, but genetics my certainly be a role in your wife's case. Certainly, weight is an issue for most, but not here. I think, down the road, we are going to understand the metabolic factors behind arthritis, but it's all very skeptical at this point.
Regarding glucosamine: There is some evidence that glucosamine/chondrotin supplements may offer benefit for knee OA, specifically. In your wife's case, it sounds as if her OA is mild, and this may be the appropriate time to introduce it.
Regenxx stem cells: I'm not familiar with the research behind this…it appears to be in the early stages of development
Unloader brace: this tends to be helpful for people who have "unicompartmental", or one side of the joint. Yes, I do find this can be helpful and can buy young people "time" before a knee replacement. There is also some evidence that the unloader brace, combines with a pulsed negative charge with a stimulation unit built in can help people delay knee replacement for quite some time.
Castor oil: Don't bother
Injectible hyalouronic acid: I've had a lot of patients have the Synvisc injections….typically consisting of 5 injections spread out by about a week or so. It's actually meant to help stimulate the growth of cartilage. Anecdotally, I'd say it' helpful to some degree in about 50% of the cases of mild to moderate OA that I see. Cortizone gets a bad rap; however, I see how effective it can be for pain management. Certainly, repeated injections done in the same spot over and over, year after year should be taken with some consideration; however, a couple times a year, to help manage chronic inflammation, it can be very effective. Sometimes, the injection is just what people need to get over their symptoms. I've seen many people fail to improve with PT, but then return to PT after having the injection and fare much better. If it were my knee, I would give it serious consideration unless I had uncontrolled diabetes (it will temporarily elevate the blood sugar).
As far as exercises, she should stay away from high impact exercise until the pain is controlled. Resistive training for the quads is also important.

My vet recommended 1/2 of a 500mg tables for my 19 lb. dog – but it's glucosamine chonhydrine, same as for humans.

Typically, Glucosamine in any form is recommended up to 1500 mg per day, usually spread out between 2-3 doses throughout the day. Depending on the condition, some doctors recommend more (sometimes 2-3 times more, but again, ir depends on the condition) and it is commonly taken with Chondroitin because those are both building blocks that your body uses to produce new cartilage.
That being said, the bottle of Glucosamine should list the recommended dosage on it. Good luck!

My herbalist aunt recommended MSM for my knee problems. When I move my leg normally, I can feel a grinding sensation when I feel on top of my knee cap. I’m only 25 and I’ve had no injuries or broken bones! I can also really feel the weather changing too and can’t stand on my feet for an extended period of time. It really kicks my hind end sometimes!
didn’t do a thing for me. I ended up having my knee scoped last week, it hurts like a bugger but Ive already got more movement than I had before the surgery. As for the walking, Id have loved to have been able to walk beyond normal around the house walking but the pain of the arthritis (mine was stage 3 osteo) kind of made that an impossibility. Vitamin D is recomended, however, if you are in the same shape as I was just to keep your mood up.
