Is it true that Caffeine improves the speed of a man’s sperm for fertility/conceiving? How much should he have?
I read it in a fertility book, is it true? Going in for my 2nd IUI today!
Caffeine and Fertility: Male
Much confusion has arisen over caffeine use in males. When sperm are treated under laboratory conditions with caffeine there is a short increase in the percent of motile sperm. However, over the longer term, the percent of moving sperm declines more quickly.
When men drink caffeine, most studies show a decrease in their ability to produce a pregnancy. The same Danish study that found a decrease in fertility from caffeine in women also found a similar decrease when men were the ones consuming caffeine.
In about 40% of infertile couples, the cause of the infertility lies with the males. There are many reasons for male infertility. For couples who are trying to conceive, it only makes sense to have an evaluation done on the males fertility health when conception hasn’t occurred after 6 months.
My husband underwent a Vasectomy Reversal in December 2000. We conceived 10 months after the reversal. His sperm count, motility, mobility, morphology, etc. were within normal range at 6 months post reversal. We had him on the supplement stated below.
There are many factors that can decrease or cease sperm production. Here are a few:
Illness
Excessive Caffeine
Stress
Excessive alcohol consumption
Street and certain Prescription Drugs
Some over the counter medications
Injury to the testicles
Blockage in the Vas Deferens

In your case I would go straight to a fertility specialist at this point. Two years is long enough to seriously consider there may be a problem. It doesn't look like you're going to get pregnant without a thorough evaluation. It could be one or both of you, so to save time I would suggest you BOTH go for testing.
A sperm analysis is relatively easy. He would provide a sample of his semen (some places have a facility, others prefer you bring in the sample within an hour of obtaining it). A semen analysis costs anywhere from $100-$200 depending on your doctor. Ours was $125.00 and that was through a fertility specialist's office. You can have your OBGYN do it if you wanted to just do that for now. But if his results are less than desirable, you'll have to seek more aggressive help. And again, it could be both of you, so I would continue with other tests when you can. It sounds like the sooner you seek help, the sooner you'll get pregnant.

Sperm banks keep semen at -321 degrees F, and they can keep it indefinitely.
There are a few options for men with a zero count. Surgical techniques like MESA and TESA can be successful with only 1 sperm, and some can even go directly into the testicle to get the sperm. So if his count is zero b/c the sperm aren't getting through the urethra during ejaculation, they could still get some viable sperm out. Here is an article on MESA and TESA: MESA.
These techinques are used in conjunction with IVF. They can be expensive (at least $12,000 per cycle with MESA/TESA).
As far as raising sperm counts, you need to determine why his counts are so low. Does he have any other biological children? It could be anything from a variocele (a swelled vessel near the testes) to a lifestyle issue (frequent hot tubbing, bike riding, sauna usage, or smoking). Your best bet is to see a reporductive endocrinologist, a fertility specialist, who can help you get to the bottom of this.
Unfortunately, you will not get pregnant naturally with such a low count, so the sooner you seek professional help, the sooner you will get your baby.
